The idea
A city population can refer to municipal limits or to an urban agglomeration, a continuous urban spread that includes adjoining units. These boundaries can give different totals for the same place. Apply the Census 2001 unit before deciding whether it crossed one million.
Why the others are wrong

(b) Ludhiana was a million-plus city.
(c) Kochi (UA) and Surat were also million-plus.
The trap
Kochi and Ludhiana feel too small, but their urban agglomerations passed a million.

A million equals ten lakh. An urban agglomeration can include a core town and adjoining outgrowths such as railway colonies or other continuous urban development. An urban agglomeration is not the same as a district or a wider metropolitan planning region; their populations cannot be substituted for one another.
